$150 per 53 - 60 minute session
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan.
I am an in-network provider for Blue Cross Blue Shield, HealthChoice, and and Aetna.
For other insurance providers, I can provide a receipt for you to file for out of network benefits.
Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How much is my co-pay/co-insurance?
- How many sessions per year does my health insurance cover?
- Is approval required from my primary care physician?
Payment
Cash, check and major credit cards accepted.
Cancellation Policy
I require 24 hour notice for appointment cancellations or rescheduling. A missed appointment fee may be charged when advance notice is not received.
